<p>Under guidance from the Principal Investigator and Project Director, provides intensive case management and coordinates clinical services for study participants receiving integrated care for hepatitis C.</p>
<p><b>RESPONSIBILITIES</b></p>
<ul>
<li>Provides intensive case management with people who inject illicit drugs to enhance entry and retention in hepatitis C medical care and treatment.</li>
<li>Coordinates care among multiple providers. Organizes case conferences and coordinates activities of a multidisciplinary team of clinicians and service providers at NDRI and other institutions.</li>
<li>Conducts HIV and HCV pre- and post-test counseling.</li>
<li>Provides ongoing education about hepatitis C, including its natural history and the risks and benefits of treatment options.</li>
<li>Conduct motivational enhancement therapy (motivational interviewing) to reduce barriers to entry and retention in hepatitis C medical care and treatment.</li>
<li>Conducts comprehensive psychosocial assessments, identifying areas of unmet needs, and develops appropriate service plans.</li>
<li>Assists patients in obtaining and using services as needed including medical care, mental health and substance abuse services, other social services, and Medicaid. Works closely with patients to ensure continuity of care.</li>
<li>Meet weekly with participants throughout antiviral treatment, providing ongoing support and adherence counseling, identifying barriers to adherence and formulating plans to minimize these.</li>
<li>Monitor for depression and other adverse events associated with antiviral treatment, and assist participants with developing strategies for minimizing treatment related side-effects.</li>
<li>Facilitates hepatitis C treatment education support group.</li>
<li>Manages ongoing collaborative relationships with community-based agencies and community partners. Ensures smooth integration of research project activities into partnering sites.</li>
<li>Contributes field information and data for reports</li>
<li>Participates in designated in-service supervision and training</li>
<li>Negotiates shared space and maintains strong working relationship with staff at collaborating community-based organizations.</li>
<li>Performs phlebotomy, processes and logs blood samples from participants as needed.</li>
<li>Assists supervisor with site related tasks.</li>
</ul>
<p><b>REQUIREMENTS </b></p>
<p><b>Education/Experience</b>:</p>
<ul>
<li>BA with 1 year relevant experience, or a high school diploma/GED plus CAC with 2 years NDRI experience as an Assistant Counselor, or high school diploma/GED plus CAC with 4 years relevant experience, or AA with 3 years relevant experience.</li>
<li>Comprehensive understanding of harm reduction approach to working with people who use drugs.</li>
<li>Must be comfortable working with people who use drugs.</li>
<li>Excellent organizational, interpersonal, writing, and communication skills.</li>
<li>Ability to function in the community and the medical center and to bridge the gap between these two environments.</li>
<li>Computer skills, including word processing.</li>
</ul>
<p><b>SPECIAL SKILLS </b></p>
<ul>
<li>Must maintain confidentiality of information.</li>
<li>Ability to demonstrate empathy.</li>
<li>Ability to build trust and maintain healthy relationships with marginalized individuals.</li>

<p><b>JOB DESCRIPTION</b></p>
<p>PACC is looking for a Substance Abuse Specialist, with CASAC certification, to join our team at Gibb Mansion. This position will provide harm reduction and educational training and counseling. The Substance Abuse Specialist ensures the quality of care for participants who have a history of substance abuse and are living with HIV/AIDS.</p>
<p><b>PRINCIPAL RESPONSIBILITIES</b></p>
<ul>
<li>Develop, facilitate and maintain substance abuse program</li>
<li>Provide individual and group substance abuse counseling, 12-step groups, peer support groups, relapse prevention programs, and harm reduction and educational training and seminars for staff and tenants</li>
<li>Develop and maintain community partnerships with treatment centers, detox and rehabilitation programs</li>
<li>Provide substance abuse case conferencing, training and consultation to all members of the social service team</li>
<li>Work as a member of the social service team in the development of recreational, socialization, educational and skill building activities with a view towards tenant inclusion and integration</li>
<li>Provide annual strategic substance abuse program work plan and keep current and accurate records</li>
<li>Provide crisis intervention as needed</li>
<li>Develop, with the tenant, and monitor individual treatment plans</li>
<li>Provide annual substance abuse assessments</li>
<li>Documentation of all services in compliance with agency standards</li>
<li>Maintain a strong working relationship with the social service staff to ensure quality of care and best practice</li>
<li>Assist tenants with accessing needed substance abuse community services</li>
<li>Evening and some weekend hours required</li>
</ul>
<p><b>QUALIFICATIONS</b></p>
<ul>
<li>CASAC certification is a MUST have.</li>
<li>Experience working with individuals with HIV/AIDS, mental health, and substance abuse issues</li>
<li>2+ years related experience in the development and facilitation of substance abuse and harm reduction training, education and individual and group work</li>
<li>Residential setting experience preferred</li>
<li>Strong written and verbal communication skills required</li>
<li>Working knowledge of various computer programs including email, Microsoft Word and PowerPoint.</li>
<li>Bi-lingual preferred</li>
<li>Bachelors Degree – LMSW preferred</li>

<p><b>PRIMARY PURPOSE: </b></p>
<p>The Outreach Counselor conducts drop-in and street-based outreach to homeless and runaway adolescents and young adults in the Haight Ashbury district, many of whom are injection drug users.  The OC promotes an understanding of the risks associated with injection drug use, including hepatitis, HIV and overdose.  Builds non-judgmental, supportive relationships with homeless youth, guides them to appropriate referral resources, and advocates for their care.</p>
<p><strong>ESSENTIAL JOB FUNCTIONS:</strong></p>
<ul>
<li>Contact and build relationships with participants in the target population during daily street outreach and staffing drop-in center and needle exchange sites.</li>
<li>Coordinate and facilitate weekly health education groups.</li>
<li>Distribute harm reduction supplies, hygiene supplies, education materials and snacks.</li>
<li>Provide preventive health education around substance use and safer sex practices, using harm reduction techniques.</li>
<li>Refer participants into counseling, mental health and medical services.</li>
<li>Refer participants into drug treatment and housing.</li>
<li>Attend staff meetings, staff groups, trainings and conferences as assigned.</li>
<li>Maintain good self-care.</li>
<li>Perform manual duties, such as cleaning, preparing snacks and stocking supplies.</li>
<li>Maintain confidentiality, appropriate boundaries and a sense of humor.</li>
</ul>
<p><strong>QUALIFICATIONS:</strong></p>
<ul>
<li><b>Personal experience with homelessness or extensive knowledge of issues facing homeless youth.</b></li>
<li>A minimum of one year of professional or volunteer experience in HIV prevention, harm reduction outreach or similar work with the target population.  Must have the ability to engage IDUs and their sex partners in informational, non-judgmental discussions about HIV, hepatitis and related health issues.</li>
<li>Must be streetwise and be able to solve practical problems.</li>
<li>Must be able to advocate for participants with medical providers, treatment providers, police, and probation officers.</li>
<li>Must be able to keep consistent records,</li>
<li>Must have a basic knowledge of Microsoft Word and basic math skills.</li>
<li>Must have good communication and listening skills.</li>
<li>Must maintain strong confidentiality and appropriate boundaries.</li>
<li>Must be sensitive to multicultural issues of race, ethnicity, gender and sexuality.</li>
<li>Must be able to work in a stressful, fast-paced environment as part of a team.</li>
<li>If in recovery, two continuous years are suggested.</li>

<p dir="ltr"><strong>Background</strong></p>
<p dir="ltr">The New York State legislature is considering landmark legislation that would require clinics and hospitals to offer hepatitis C testing to all people born from 1945 through 1965 &#8212; “baby boomers” &#8212; in accordance with new CDC guidelines.</p>
<p dir="ltr">Baby boomers make up the majority of people with chronic hepatitis C, but most remain undiagnosed. New York State can lead the nation in responding to the hepatitis C epidemic. This legislation is a major first step, but we need your help to get it passed. Our form allows you to contact your State Senator and enlist their support.</p>

<p>Harm Reduction Coalition is a national advocacy and capacity-building organization that promotes the health and dignity of individuals and communities impacted by drug use. We are now seeking a full-time <strong>National Conference Organizer </strong>to join its growing team.</p>
<p>As a critical member of the staff, the position supervises and implements our most visible and well-attended program: the <em>National Harm Reduction Conference</em>. The position is responsible for event logistics and coordinates the efforts of agency staff and harm reduction advocates from all over the US to design a relevant agenda driven by our community.</p>
<p><strong>Responsibilities</strong></p>
<p><span style="text-decoration: underline;">Event Planning</span></p>
<ul>
<li>Create a Conference Organizing Timeline and ongoing status reports; develop standardized checklists for conference needs and to ensure replicability in future events.</li>
<li>Oversee drafting and graphics development of all conference materials including Save the Date Postcard, registration booklet, Invitation to Exhibit, registration form, co-sponsorship letter, confirmation postcards, and agenda booklet. Coordinate with Harm Reduction Coalition’s graphic design consultant.</li>
<li>Create a targeted outreach plan that will augment the numbers of under-represented communities who attend and present at the National Harm Reduction Conference.</li>
<li>Recruit and manage volunteer corps at the conference.</li>
<li>Ensure that all inquiries regarding the National Harm Reduction Conference receive a timely response.</li>
<li>Plan and coordinate conference events beyond the main agenda including but not limited to the Conference Film Festival, special services such as free reiki, massage, and child care.</li>
<li>Coordinate all materials (bags, flyers, badges, brochures, etc.) for shipment to conference venue.</li>
<li>Supervise conference assistant</li>
<li>Oversee and maintain relationship with the conference host hotel including A/V, refreshments, room commitments and communication with hotel staff.</li>
<li>Collaborate with Harm Reduction Coalition’s preferred A/V company.</li>
<li>Organize travel logistics for staff and conference attendees.</li>
<li>Oversee the scholarship application and award process.</li>
<li>In collaboration with Evaluation Manager, coordinate evaluation development, distribution and collection at the conference, as well as data entry of demographics and presentation evaluations.</li>
</ul>
<p><span style="text-decoration: underline;">Conference Program Planning</span></p>
<ul>
<li>Develop strategic relationships with harm reduction community-based agencies and SEPS to foster the involvement of Program Committees that will direct tracks and topics presented in Baltimore 2014, assist in acquiring CEU credits for conference participants, review abstracts and propose innovative learning methodologies.</li>
</ul>
<ul>
<li>Direct efforts with Local Organizing Committee members and Harm Reduction Coalition staff to articulate the conference theme and maintain consistent and integrated messaging throughout marketing materials, web design, exhibitor packages etc.</li>
</ul>
<ul>
<li>Oversee and contribute to setting of the conference agenda and topic tracks.</li>
</ul>
<p><span style="text-decoration: underline;">Fundraising and Finance</span></p>
<ul>
<li>Collaborate with the Development Manager, Executive Director and Director of Operations and Administration to fundraise from corporations, foundations, and surrounding vendors to sponsor Conference activities.</li>
</ul>
<ul>
<li>Develop a conference budget based on previous year’s expenditures; maintain documentation of expenditures and review variance reports with the Finance Department quarterly.</li>
</ul>
<p><strong>Qualifications</strong></p>
<ul>
<li>Minimum Bachelor’s Degree</li>
<li>Previous conference organizing experience</li>
</ul>
